I actively engage in therapeutic activities tailored to cognitive abilities, ensuring a supportive environment. Collaborating with the healthcare team, I communicate observations, contribute to care planning, and maintain accurate documentation. Through ongoing self-reflection, I refine my skills to provide compassionate and specialised care that addresses the unique needs of geriatric mental health patients.
• COMMUNITY FOCUS-PD RENAL AND MAUR RENAL SERVICES, 02/27/23 - 03/29/23, During my community Renal and MAUR clinical placement, I've acquired essential skills in managing dialysis equipment and procedures, assessing vascular access sites, monitoring fluid and electrolyte balance, and ensuring infection control measures. Additionally, I've developed the ability to educate patients about renal care, medication management, and dietary restrictions. Collaborating within the interdisciplinary team and communicating effectively with patients and their families are integral components of my skillset.
• MENTAL HEALTH FOCUS-PATHWAYS MENTAL HEALTH FACILITY, 09/05/22–10/06/22, During my community mental health clinical placement as a student nurse, I have acquired a range of essential skills, including:
• Therapeutic Communication: I have learned to engage in active listening and effective communication techniques, enabling me to build rapport and establish trusting relationships with clients in the community mental health setting.
• Assessment and Care Planning: I am now proficient in conducting comprehensive mental health assessments, recognising signs of mental health issues, and developing individualised care plans to address clients' unique needs.
• Mental Health Promotion: I have gained insight into the various approaches to mental health promotion within the community, including organising awareness campaigns and educational sessions to reduce stigma and enhance mental well-being.
• Medication Management: Through collaboration with healthcare providers, I have learned about the proper administration and monitoring of psychotropic medications, understanding their indications, side effects, and potential interactions.

• GENERAL SURGERY WARD 9, 03/02/22 - 04/01/22, During my general surgery clinical placement, I applied the skills I learned by actively participating in patient care, assisting in clinical procedures, and collaborating within the healthcare team. I effectively communicate with patients, provide education, and document care accurately. Continuous self-reflection and adaptation further enhance my abilities to contribute positively to surgical care.
