  • Please ensure all medical conditions are declared when you complete your child’s online enrolment.
  • SCHOOL LUNCH DIETARY REQUIREMENTS
  • A free lunch is supplied to our students each day. The portions are moderate - your child may still need to bring extra food from home. Please ensure that any dietary requirements are included as medical conditions e.g. food allergies etc
  • In submitting an online enrolment for my child:
  • I give permission for school staff to administer first aid treatment to my child when necessary.
  • I give permission for school staff to administer non-prescription medicines (e.g. paracetamol, antihistamine), on occasions deemed necessary.
  • I give permission for school staff to act on my behalf in the event of an emergency, and I agree to meet any costs incurred.
  • I will inform the school if there is a change in my child’s health care needs.
  • I will inform the school when my contact details change so I am able to be reached in the case of a medical emergency involving my child.

EOTC – EDUCATION OUTSIDE THE CLASSROOM

  • In submitting an online enrolment for my child:
  • I give permission for my child to participate in all EOTC events on site in the school grounds (Category A), and off site events in the local community within school hours (Category B). I understand that in these instances a permission slip may not be required by the class teacher.
  • I understand a separate consent will be sought from the school for all EOTC events outside of school hours, or outside of Hamilton (Category C), and off site residential overnight events (Category D), or any activity that might also be deemed to be of higher than normal risk.
  • I have provided the school with up to date medical, supervision and learning information through this enrolment form and will make every endeavour to keep this information current.

INFORMATION & COMMUNICATIONS TECHNOLOGY (ICT) – BRING YOUR OWN DEVICE (BYOD)

In submitting an online enrolment for my child I agree that I have read, understood, and explained to my child the following Peachgrove Intermediate ICT Responsible Use Agreement for Students and Parents, as well as the BYOD User Agreement, and my child and I endorse and agree to abide with the parent declaration and BYOD User Agreement provided.

PARENT/CAREGIVER DECLARATION

  • In submitting an online enrolment for my child I agree that:
  • The information provided in the online enrolment form and other related information provided by families is required to enable the school to comply with its functions under the Education & Training Act 2020 and other enactments.
  • The information may be used for school enrolments and academic progress, maintaining and up-dating records, providing additional academic and personal support, general administration, and supporting student welfare.
  • Some agencies where disclosure is required for the maintenance of Law and Order as defined in Principle 11 of the Privacy Act 1993 may be notified.
  • I give permission for my child’s records to be obtained from their previous school, and their records to be sent on to their next school after leaving Peachgrove Intermediate.
  • I give permission for the school to access my child’s immigration documents, if necessary.
  • I give permission for my child’s photo to be published in school marketing and/or social media sites.
  • I agree to abide by the rules and regulations of the school, and to pay the appropriate fees.
  • I will make sure that my child will be punctual, wear correct uniform (clearly named), and abide by the school’s expectations as per the following Peachgrove Pride and Positive Behaviour for Learning Plan.
  • I declare that all information provided in this enrolment form is true and correct and I will advise the school office should there be any changes to what I have provided.

PEACHGROVE PRIDE and POSITIVE BEHAVIOUR FOR LEARNING

  • At Peachgrove Intermediate School, we believe every teacher has the right to teach and every student the right to learn in a safe, secure environment, uninterrupted by the behaviour of others.
  • When students arrive at Peachgrove Intermediate, they learn about our school's PRIDE values, which are about being:
  • Positive in attitude
  • Respectful of self, others and property
  • Inclusive of all
  • Determined to do our best
  • Engaged in our learning
  • These 5 values set the tone for the way in which we expect all members of the Peachgrove community to operate. Our students learn what these values look, sound and feel like in different settings around the school, and are supported to display them at all times.
  • This assists us to create a culture where positive behaviour allows learning to thrive. Students who meet the school-wide behaviour expectations or demonstrate our school values receive positive consequences and acknowledgements to encourage those behaviours to continue. Students who do not meet the expected standards of behaviour are supported by their classroom teachers, Team Leaders and/or member of the Senior Leadership Team to develop the behaviours required.
  • Constant or Major Misbehaviour
  • Teachers employ a range of strategies to support students to display appropriate and desired behaviour. If the student does not respond to these interventions then contact will be made with the parents/caregivers to inform them of this.
  • Constant misbehaviour or reoffending in the playground or classroom is not acceptable, and the school’s leadership team will determine, after consultation with teachers and parents/caregivers, what interventions/consequences are most appropriate in each case to best address the behaviours.
  • Where the behaviour involves a breakdown in relationship, all efforts will be made to restore the relationship in accordance with the principles of restorative practice.
  • Our policy as a school is to consider the best interests of all concerned and to maintain the dignity and mana of all.
  • Serious Misbehaviour
  • If a student is referred to the Senior Leadership Team for serious misbehaviour, the principles of natural justice will apply, and the school will act fairly and reasonably in the circumstances. In some situations, this may involve the school requiring the student to remain at home while the school investigates all avenues of additional support. In this instance, the school will provide the student with a suitable programme of work. If the matter is serious enough, a stand down or suspension may have to be considered.
  • Should you have any concerns over the behaviour of your or another child, we encourage you to contact your child's classroom teacher in the first instance. Any matters of a serious nature should be referred to the Principal, Deputy Principal or Assistant Principal.

ICT Responsible Use Agreement Student and Parents

  • When using information & communications technologies (ICT) at Peachgrove Intermediate School I will always be a good digital citizen. This means that I:
  • 1. will be a confident and capable user of ICT.
  • I know what I do and do not understand about the technologies that I use. I will get help where I need it.
  • 2. will use ICT for learning as well as other activities.
  • I understand that technology can help me to learn. I also know it can also be used to talk to people, to buy and sell things and to have my opinion heard. I know when and where it is OK to do each one.
  • 3. will think carefully about whether the information I see online is true.
  • I know that it is easy to put information online. This means that what I see is not always right. I will always check to make sure information is real before I use it.
  • 4. will be able to speak the language of digital technologies.
  • When people talk online the things they say can be quite different from a conversation they might have if they were sitting next to each other. I know that I must try to understand what people are saying before I react to them. If I am not sure, I can ask them or someone else to explain.
  • 5. understand that I may experience problems when I use technology but that I will learn to deal with them.
  • I understand that there will be times when technology may not work as I expected it to, or that people may be mean or unkind to me online. When these things happen, I know that there are ways I can deal with it. I also know there are people I can go to, to get help if I don’t know what to do next.
  • 6. will always use ICT to communicate with others in positive, meaningful ways.
  • I will always talk politely and with respect to people online. I know that it is possible to bully or hurt people with what I say and do on the internet. I will think about the effect that my actions have on other people.
  • 7. will be honest and fair in all of my actions using ICT.
  • I will never do anything online that I know will hurt anyone. I will make sure what I do is not against the law. I will make sure that my actions don’t break the rules of the websites that I use. When I am not sure about what I am doing I will ask for help.
  • 8. will always respect people’s privacy and freedom of speech online.
  • I understand that some information is private. I will be careful when using full names, birthdays, addresses and photos of other people and of my own. I also know that I will not always agree with what people say online but that does not mean that I can stop them or use it as an excuse to be unkind to them.
  • 9. will help others to become better digital citizens.
  • Being a good digital citizen is something that we all have to work at. If I know that my friends are having problems online, I will try to help them. If I see that someone is being unfairly treated online then I will speak up rather than just watch it happen.

Parent Declaration

  • I understand that our school is supporting students as they learn the skills required to become successful digital citizens. Our school defines a digital citizen as someone who:
  • Is a confident and capable user of ICT
  • Uses technologies to participate in educational, cultural, and economic activities
  • Uses and develops critical thinking skills in cyberspace
  • Is literate in the language, symbols, and texts of digital technologies
  • Is aware of ICT challenges and can manage them effectively
  • Uses ICT to relate to others in positive, meaningful ways
  • Demonstrates honesty and integrity in their use of ICT
  • Respects the concepts of privacy and freedom of speech in a digital world
  • Contributes and actively promotes the values of digital citizenship
  • I understand that our school has a policy and associated procedures
  • Which outlines the school’s digital citizenship approach and how this supports teaching and learning.
  • These documents are available on the school website / on request / at the school office.
  • I understand that the school provides access to the internet and other communications technologies because it believes that they enhance the teaching and learning process.
  • I understand that the school encourages responsible behaviour by students when using technology to help protect themselves and each other from experiencing harm online. I am aware that this “Responsible Use Agreement” is part of that, and that it encourages students to consider how their actions can affect themselves and those around them.
  • I have read the student declaration and have talked with my child about what it means to them. I am happy that my child understands what this means, and that they are capable of working within the guidelines.
  • I am aware that students can experience challenges when using technology, but that the school makes every effort to support students to manage those challenges effectively. I understand that by learning to deal with them in a safe environment with the appropriate support they will be better prepared to deal with those challenges in the future.
  • If I have questions or concerns about the way in which technology is being used by my child at school, I know the school is happy to discuss this with me, and I am aware that I am welcome to do this at any time.
  • I also understand that on occasions, a photo of my child or a sample of their work may be included in school publications (e.g. winning team photo in a school newsletter) as a means of promoting activities and learning.
  • In these instances the school policy is to include only a student’s first name and room number.

Bring Your Own Device (BYOD) User Agreement

  • 1. I want my child to bring his/her own digital device to school for use in the classroom programme in order to help him/her in the learning process.
  • 2. I understand that:
  • Any device brought to school by my child is his/her responsibility and the insurance for the device is my responsibility (as his/her parent/caregiver).
  • Teachers will provide a locked cupboard in which my child may place his/her device when not in use.
  • All devices must arrive at school each day fully charged.
  • Devices are only to be used with teacher permission.
  • Devices brought to school are for the use of my child.
  • Any sharing of such devices is at my child’s discretion.
  • Teachers will encourage the use of digital devices whenever possible and that students will be able to use them for homework activities.
  • Students must use cloud based applications such as Google Apps, or their own device capacity, for file storage and management.
  • Any guidelines and rules, including the Digital Citizenship policies, governing the use of any school owned digital devices will apply to student owned digital devices, while they are at school, including adherence to the Copyright Act.
  • 3. I give my permission:
  • For school authorised staff to check my child’s device(s) when necessary, to ensure that the device(s) is free of viruses or any other issues that make the device(s) unsuitable for school use.
  • If such a concern is verified, I understand that the device will be returned to me to get it repaired. I also understand that the device(s) will be audited before my child is able to use it again on the school’s premises.
