  • Service coordinator and I work alongside actively by advocating people in terms of independence, equality and self-esteem to be able to participate in aspects of everyday life
  • This includes a yearly review of outcomes/ goal plans together with the client, family members and service coordinator
  • Writing a monthly/ initial report and submit to service coordinator to monitor all the goal plans are on the right track
  • Submit event and incident report to service coordinator when there is risky or challenging behaviour occurs
  • Involved with DHB nurses, doctors and service coordinator together to come out effective solutions to support challenging clients
  • Worked with clients around building their daily living skills communication/ social skills (improvisational storytelling and modelling), literacy skills (practice reading story and writing sentence), independent living skills (taking public transport, purchasing and money handling), community/ recreational participation (Zoo, library, parks and art class) and engage physical activities (dancing and walking)
  • All learning skills are variable day by day and are based on the client's daily health and emotional condition
