  • In my role as Care Manager, I was responsible for my own caseload, which incorporates the following: Undertaking overview assessments of adults with learning disabilities to establish their residential, financial and social needs to ensure appropriate services were offered and assessed needs are met within departmental deadlines
  • To undertake assessments of those who care for adults with learning disabilities to ensure their needs are met in accordance with The Recognition of Carers Act
  • To develop, implement, review and monitor care plans of clients and carers as their allocated Care Manager Communicating within a Multi-Disciplinary-Team to encourage partnership working within planning and co-ordinating individual care packages Ensuring the confidentiality of information within all aspects of verbal and written information and documentation is adhered to in accordance with the Data Protection Act
  • To participate in job related training Assisting clients with independent living skills and daily activities within the community
