Femi Thomas

I provide proven expertise in managing frailty / care of the elderly patients within a home environment under the Living Well (the five Wellness Pillars) model of care. I provide management support to the wider inter-professional team in coordinating individual social and health care needs. An essential element of my role is evaluating and implementing rehabilitation plans in conjunction with the client and their relatives: ensuring family centered care. A focus has been on promoting health and wellbeing in self-management frameworks within an Aged Care environment. I supported the safe discharge of patients, post rehabilitation, home which included arranging and leading case conferences, referral processes, and ensuring that ongoing support was in place. I continue to develop my knowledge base in chronic disease management and palliative care. My career goal is to enter the Nurse Practitioner pathway; I was advised by the Post Graduate Dept to continue to develop my clinic experience within NZ.
