• The Nursing care Unit has 30 bedded residential geriatric and palliative care center
  Provide care and support to residents who have long-term geriatric issues.
  Responsible for assessing, preparing reviewing patient care plan, including medication
• Observe and report changes in the client's behaviour and presentation promptly to the casemanager and the psychiatrist to review their mental health, medication, and safety plans
• Medication, social, pastoral and cultural support to those in the r service having mental health and geriatric issues
• Suggesting relevant therapies and social activities for the client's wellbeing
• Build effective relationships with service users and their whanau, and inform family about
  any changes in the client's needs and progress.
• Proper recording and reporting of identified problems and risks Liaise with team of doctors, patients, family and Multidisciplinary team (physiotherapists, pharmacists, occupational therapists etc) as indicated
• Assisting admission and discharge procedures
• Prepare Care Plan for residents and do periodic review, ensure support given as per care plan health and safety, infection prevention, and client files.
• Assess risk for fall/accident, plotting with Braden scale
  Assist residents with personal care tasks, including Manual handling, hygiene, grooming, toileting,mobilizing, shaving, and dental and hair care
  Daily taking and recording vitals of patients and if any changes reports to doctors
• Assist with ADLS, Administer prescribed medication/injection and blood extraction
• Nutritional assessment ad administers special feed as indicated, PEG feeding
• Attending hygienic needs of patients
• Collection various biological samples for pathological and diagnostic tests
  Emphasis on infection control and using aseptic technique where required
  Bring residents for doctors appointment
  Make necessary arrangements for referral to specialist doctors
  

• Report to the clinical nurse manager and worked under supervision of RN and preceptor
• Provide care to residents following care plan, cultural safety code of rights of health consumers legislation and health care policy under New Zealand
• provide support for personal hygiene ,support activities as per care plan
• Documentation of daily activities and support
• Learned to apply Treaty of Waitangi Principles Participation, Partnership, Protection, at Health care settings
• Learned and ability to apply code of health and disability rights for residents
• Prompt reporting of any health and safety hazards as per organisation policy
• Apply Critical thinking, assessed clinical skills, cultural safety during nursing practice

• Collaborate with multidisciplinary health care professionals for providing best
