· Establish and maintain healthy and positive relationships with residents.
· Ensure the resident fully understands the role of the Support Worker and feels encouraged and comfortable seeking contact and assistance.
· Actively strengthens relationships with team members and management.
· Promote and facilitate two-way working relationships with key stakeholders and other community groups.
· Demonstrates positive working relationships with residents and family/Whanau. Maintains clear and concise records and acts to resolve any potential issues.
· Reports any risk or concerns to the Line Manager. Responsible for assessing clients’ support needs through focussing on their strengths, abilities and resources and how best to enhance these to achieve the clients’ goals and support recovery.
· Responsible for working collaboratively with clients and their families/whanau to access information, education, resources and support to develop skills and independence in the community of their choice.
