Over the last ten years, I have worked in the area of mental health, supporting a wide range of tangata whaiora in the community, with a particular focus on supporting rangatahi (youth). Whilst I consider the role to be an honour and a privilege, I believe it is time for the next step in my career. I bring 7+ years of experience working in the area of mental health and addictions with individuals across varying demographics to promote healthy living.

Community Support Worker – Access & Choice

  • Receiving email referrals on a weekly basis and using a range of databases as required, including MedTech to communicate directly with GPs, nurses, HIP and Health coach for their patients that require social supports, etc
  • Meeting tangata whaiora and establishing a plan with them for interim support
  • Sourcing information, networking and referring tangata whaiora to relevant services with their consent
  • Providing transport/supporting whaiora with their appointments if required
  • Networking and advocating for whaiora, as needed.
