• Assess mental status and level of consciousness
• Observe for neurological deficits with frequent and serial neurological assessments with aim to prevent further complications
• monitor vital signs, administered injections, managed medications, and provide medical procedures
• collaborated with the multi-disciplinary team of physicians, nurses, therapists, social workers and other healthcare professionals to develop individualised treatment plans for each patient
• provide post stroke rehabilitation and promote patient independence
• assist with activities of daily living
• educate patients and family about stroke recovery and prevention
• co-ordinated discharge planning activities including arranging follow up appointments or referrals for additional services
