MOE TOFILAU

This is a child centred, intensive home visiting programme that focuses on improving children's health, learning and relationships, family / whanau circumstances, environment and safety.

My role was

• Support vulnerable children and reduce maltreatment
• Promote health and education outcomes.
• Facilitate change to put child's safety at fore front
• Ensure that all families/whanau are aware of the health and development needs of their children
• Encourage children and adults to communicate and understand their their world best within the context of their own language and cultural traditions
• Attend meetings with OT and court hearings if required to

• Provide counselling sessions for individuals and groups / families/whanau
• Perform initial evaluation of clients and develop customised treatment plans
• Building relationships of trust and respect with clients, explaining involved documents and their entailments before signing.
• Help clients define goals, plan actions, and gain insight
• Develop relevant therapeutic processes
• Take a holistic approach to mental health care
• Refer clients to other services if need to
• Outline follow up process
• Update all information onto database
• Submit report to manager monthly and whenever required
• Attend in house and community trainings and meetings.

• Receive referrals from hospitals, medical clinics, prison, public
• Organise initial meet and greet
• Provide specialised advice to smokers and good practice to help them quit smoking - face to face and on radio
• Facilitate rehab care programme for service users from prison
• Support mental health service users around tobacco addiction
• Provide evidence-based approaches in line with national guidance
• Submit monthly report to Manager

• Safety and well being of residents is top priority
• Receive handover from previous shift
• Allocate residents and responsibilities to team members
• Carry out hygiene and personal cares according to residents plans
• Give medication and record on database
• Perform diversional therapies / interact with residents / engage with their families / whanau
• Enter all notes on database and update manual files
• Other duties as directed from time to time
