Specific Task : Provides assessment through daily blood sugar monitoring by use of glucometer with record of values taken and autonomously gives DM medication as prescribed by family medicine doctor. Monitors vital signs 3x a day to ensure it is within normal limits detects any abnormality and reports to doctor during routine check-up. Takes part in the optimal dietary needs of patient to ensure intake is within healthy standards parallel to underlying condition. Encourages passive range of motion exercises to ensure active blood circulation.
2.Female Brain Cancer patient with Bone metastasis terminal stage already in palliative care, bed ridden with Indwelling foley catheter connected to urine bag, dependent on nasogastric tube feeding and maintenance of oxygen inhalation 2lpm/day, with selective verbal response.
Other underlying condition: hypertensive, DM type 2, altered neurologic and mental state
Specific Task: Private Duty Nurse is on rotating shift with endorsements done and active Nursing progress notes. Nurse on duty needs to discuss full management with consideration of palliative care as per request of family members on daily basis.
1.Ensures Vital signs monitoring before start of shift. Performs routine check on patient contraptions like O2 inhalation dependent on result of O2 saturation by the use of pulse oximeter, NGT must be intact, IFC is noted to be with free flowing urine with countercheck measure on the intake and output charting.
2.Check positioning of patient and note that turning has been done, note for skin integrity. Physiologic needs of the patient must be met.
3.Sufficient rest and comfort is promoted.
4.Prepares NGT feeding by osteorizing fruits and vegetables as organic food supplementation per advice by dietitian. Administers feeding observing safety protocol initially, tube test is performed to ensure no blockage and placement is correct.
5.Intake and output chart is initiated to observe equilibrium on patient’s body system.
6.Suctioning, nebulization and bronchopulmonary tapping is rendered as needed depending on patient’s condition.
7.Bed bath, douching, and eye toilet are part of morning care procedures done to ensure physical hygiene is promoted. Evening care is promoted by mild sponging and douching for comfort.
8.Practices changing of NGT tube every 7th day to ensure aseptic principle and IFC changing every 14th day to prevent infection.
