  • Provide change leadership and facilitation to support the national rollout of the Health Care Home (HCH) Model of Care and Digital Health projects across the Primary Health Sector.
  • Developed and maintained relationships with key stakeholders and member organisations consisting of Primary Health Organisations, General Practice and other associated Health organisations.
  • Worked closely with the Collaborative Aotearoa team and associated network to deliver projects and content to drive enhancement of equity and the health and wellbeing of the Aotearoa community and Whānau (family).
  • Workshop, support and present customised material on the Model of Care and LEAN Change Management to General Practice and PHO to promote uptake and utilisation of the program, virtually and in person.
  • Workshop, support and present Collaborative Aotearoa programs at the 2023 Hawkes Bay Symposium and the 2023 PMAANZ Conference as well as various webinars and regional events.
  • Support members by providing orientation, mentoring and on-going support in the Collaborative Aotearoa space alongside connection between member organisations to enhance and support their PHO service delivery.
  • Organise and lead bi-monthly Member Meetings online and in person creating a space to connect, collaborate and brainstorm to improve the HCH Model of Care and how to support Primary Care.
  • Organise and facilitate monthly Peer Learning Sessions to PHO, General Practice Networks, Healthcare Professionals and interested Community members with relevant and current speakers and content.

Health Care Home Lead/Change Facilitator

  • Joined the 'Health Care Home' (HCH) Project as Change Facilitator/Administrator. Within 6 months progressed to Co-Lead/Change Facilitator.
  • Using the HCH Model of Care and Building blocks facilitated 12 General Practices in the Otago rohe through their HCH Journey to improve quality and sustainability of services as well as patient and staff experiences.
  • Engaged with Practice Teams in the HCH Journey to set measurable achievable goals, guide them through changes/improvements, troubleshooting issues, right through to completion.
  • Tracked and updated budgets, contracts, monthly practice funding and target payments, ensuring all were transparent and accurate.
  • Organise and Facilitate monthly Steering Groups to approve HCH Practice Implementation Plans, End of Year reports and release of end of project funding.
  • Using my Practice Manager experience and HCH knowledge worked alongside the Practice Network Team to support Practices in their general day to day running and implementation of WellSouth programs including Foundation and Cornerstone Accreditation.

  • Managed the day to day running of the centre under the guidance of the 4 Directors and alongside the Financial Manager.
  • Worked closely with two Clinical Lead Nurses and Clinical Support Team leader to supervise and support the team of 10 doctors, 11 nurses and 8 admin staff.
  • Co-lead of Health Care Homes team. Successfully completed the 3 year project and went on to become an Accredited Health Care Home Practice through Collaborative Aotearoa.
  • Presented "The Waiting Game" at the National Collaborative Aotearoa Symposium. This outlined the changes made through the HCH Program to successfully reduce wait times throughout the medical centre.
  • Organised some of the first GP lead COVID-19 vaccination clinics which ran daily, including a mass vaccination clinic during Level 4 lockdown. Have since received an Individual COVID-19 Response Recognition Award.
  • Assessed and updated organisational policies and procedures in line with the Cornerstone Accreditation 2021 requirements.
  • Ran a Patient Engagement Group to hear the 'Whanau Voice', implemented appropriate changes.
  • Addressed and remedied all patient and team member issues/complaints.
