RELEVANT CLINICAL NEEDS ASSESSMENT EXPERIENCE

  • My experience across acute hospital and aged residential care provides me with a broad understanding of the journey of older people through the health system. In my current Clinical Manager role, I regularly work alongside Needs Assessment services in relation to assessment, support needs and placement of residents. This has given me practical insight into the importance of identifying clinical, functional, social and environmental needs when determining appropriate services and levels of care.
  • I am competent in NZ InterRAI and understand how assessment information supports care planning, identification of risks and changing levels of support. My Nelson Hospital experience has strengthened my ability to assess acute changes in health, understand complex conditions, contribute to multidisciplinary decision-making and support safe discharge and transitions of care.
