  • First and foremost, acting as an advocate for our patients and their specific needs to provide them with support, information and tailored care
  • Working as an intermediary for our patients between their case managers, support workers, district nurses, occupational therapists and social workers. Helping them to seamlessly navigate their varied care teams.
  • Scheduling daily rosters, coinciding with patient needs in a time pressured environment
  • Liaising with support workers on their daily rosters and patient clinical concerns
  • Building rapport with both clients and their family regarding concerns or change of requirements and needs
  • Using our client management system CRM and Microsoft Excel daily to keep client care information up to date
